AI in Value‑Based Care:
Where the Continuum

The Executive Question

Why do some AI-enabled VBC investments convert into measurable financial performance, while others remain clinically impressive pilots that never reach Value Capture?

01  The Value Continuum

The framework this entire brief is built on

What is the Value Continuum?

Health Kinect's 12-dimension framework for evaluating any AI healthcare product — from how it's built, to how it's delivered, to how it's actually monetized. Every judgment in this brief traces back to where a tool, vendor, or use case sits along these four stages. Adapted from Massa, Tucci & Afuah's business model research (Academy of Management Annals, 2017).

02  The VBC Paradox

Intelligence ≠ Action

AI can identify risk, detect care gaps, and recommend the next best action with increasing precision — strong Value Creation, in Continuum terms. None of that guarantees the recommendation gets captured as actual behavior change. That gap is where most value-based care initiatives quietly stall.

03  Skin in the Game

Incentives are the layer AI can't substitute for

Across payer performance data and practitioner conversations, one pattern holds: AI amplifies incentives that already exist. It doesn't create new ones. Where providers carry real financial risk — a Value Capture question — AI-enabled insight gets acted on. Where they don't, it gets filed away.

Two-sided risk performs best

Providers in two-sided risk arrangements showed the strongest results across functional status, blood pressure control, blood sugar management, and cancer screening — outperforming upside-only models.

Everyone has to row together

The highest-performing VBC organizations align incentives end-to-end — physicians, support staff, and care teams all financially tied to the same outcome measures.

"Without belief in the mission and a financial stake, behavior won't change — AI or no AI."

04  Two AI Value Equations

There are only two honest ways AI creates value

Every AI-in-VBC pitch eventually reduces to one of two equations — both a Value Creation claim. Knowing which one you're being sold is the fastest way to evaluate a vendor.

Equation 1
Same Outcomes
÷ Lower Cost
Production Efficiency
+
Equation 2
Better Outcomes
÷ Same or Lower Cost
Clinical Improvement
The ideal outcome captures both, at once

05  AI Market Map

Startups and incumbents, competing on different ground

The question worth asking isn't who's "winning" AI in VBC — it's where each player is actually placing its bets on the Continuum.

CareTrak®

Risk stratification across 60+ EHRs; 85% efficiency gain vs. manual review.

Innovaccer

Unifies fragmented panel, analytics, and care-management views.

RAAPID

Risk-adjustment intelligence tied to financial performance.

Persivia CareSpace®

Built for the CMS TEAM Model's episode-based accountability.

Epic

AI Charting, Penny, native VBC analytics, 305M+ record HIE.

Oracle Health

Clinical AI Agent (40%+ less documentation time).

Veradigm

Payer Analytics for risk adjustment, 20,000+ practices.

athenahealth

athenaAmbient plus a VBC analytics assistant, 170K providers.

06  Where the Market Clusters

Everyone is strong on Creation. Almost no one owns Capture.

Plot the eight vendors from the market map onto the Continuum, and the pattern is unmistakable: the crowd sits in Value Creation — the technology engine. Almost none have a proven Value Capture model built for risk-based economics.

White Space

Almost nothing sits in Value Capture — a pricing model built for two-sided risk, not fee-for-service. That's not a technology gap. It's an incentive gap: a recommendation only becomes revenue once someone is actually paid to act on it, and most contracts still don't pay that way.

07  The Missing Layer

Closed-loop action is the frontier

Most AI in VBC stops at prediction — a dashboard, a flagged patient, a risk score. The frontier is a loop that doesn't stop until the action is taken, measured, and improved. Incentives sit at the hinge, between recommendation and action.

The Hinge

"Act" is the step every dashboard skips and every incentive structure decides. Closing the loop means designing the incentive into the workflow, not bolting AI on top of it.

08  Next Frontier

Can AI lower the cost of staying independent?

Independent practices are structurally locked out of risk-based contracts by the same infrastructure costs incumbents absorb at scale. AI may be the first lever that changes that math.

49%

38%
cite financial/risk exposure as the top barrier
88%
believe AI/automation could meaningfully improve practice efficiency
44%
of U.S. PCPs still receive no value-based payments at all

09  The Health Kinect View

Three conclusions

01

Incentives beat intelligence

AI can recommend action. Alignment determines whether it happens consistently.

02

Efficiency is real value

Maintaining outcomes at materially lower operating cost matters — it doesn't need to be dressed up as more than it is.

03

Closed-loop action is the frontier

The opportunity is intelligence that continuously drives, measures, and improves action — not another dashboard.

“The goal isn't AI‑enabled healthcare.
It's better healthcare enabled by AI.
